Job summary

Hampshire and Isle of Wight NHS Foundation Trust has an exciting opportunity for an experienced Nurse to join a fast-paced supportive team based in a primary care environment. The successful candidate will conduct home visits to patients on behalf of the GP to assess and initiate treatment in their own homes or care homes. The successful candidate will work alongside our paramedic, physio and occupational therapists in our frailty team to provide proactive care management, future care planning and holistic assessments based on complex geriatric assessment.

We are looking for an experienced nurse with a commitment to providing compassionate care that responds to individual needs. The successful postholder will have experience in working autonomously, and as part of a wider multidisciplinary team, undertaking physical assessments for patients who are acutely unwell and need on the day assessment and treatment.

The post holder will be passionate about providing proactive care and support to prevent hospital admission and promote independence in the community environment.

The service is delivered from 8am-6.30 pm Monday to Friday. In this role you will work Monday, Wednesday and Friday, covering either 8-4 or 10.30-6.30 as per the needs of the service.

Main duties of the job

*Use existing knowledge, expertise and enhanced levels of clinical judgement to independently undertake complex and holistic assessments

*Critically evaluating and analysing clinical interventions to make evidence-based decisions on how to manage the delivery of complex clinical care.

*Caring for people with complex care needs using their existing knowledge and expertise, enhanced clinical assessments, diagnostics, interventions and equipment. They must have the confidence and ability to think critically, and to apply a depth of knowledge and highly developed skills to provide enhanced evidence-based care

*Working as part of a wider health and care team whilst being able to work alone when seeing people in their own homes. They provide complex clinical care in the context of continual change, challenging environments, different models of care delivery, an older and more diverse population.

*Negotiating boundaries and playing an important proactive clinical leadership role in multidisciplinary teams to support the integration of health and social care services

*Delegating work to other members of the multidisciplinary team and taking accountability for that delegated activity.

*Act independently to plan, deliver, monitor and evaluate care using enhanced clinical assessments, diagnostics, and interventions

*Be an accountable professional acting in the best interests of people, putting them first and providing clinical care that is evidence-based, person-centred, safe and compassionate.

  • Professional UK registration e.g. NMC / HCPC
  • Degree level award

  • Able to demonstrate a good command of the English Language both written and verbal
