Job summary
To be responsible for the operational management of Community Independence Service (rehabilitation and social care staff) as part of the Locality Health Team, with responsibility for ensuring safe staffing levels, optimum use of resources and continuity of high quality service delivery.
To maintain a clinical caseload, whilst providing supervision and support for clinical staff day to day, providing specialist clinical knowledge for complex cases and collaborative working with colleagues in other primary, community secondary and health and social care organisations.
Work as an accountable and autonomous senior clinician pro-actively assessing, treating, monitoring and anticipating changing needs of patients who have complex/and or chronic presentation to improve health outcomes and manage risk by increasing support enabling them to remain in their home/community and reducing avoidable hospital admissions.
This is a full time role (37.5 hours per week) working from the Royal South Hants Hospital.
Main duties of the job
Maintain an active caseload of patients, identified by self, community, primary and secondary care colleagues, using a range of predictive tools and clinical assessment with the expectation that time is divided appropriately between clinical and operational management.
Maintain clinical skills and expertise focusing clinical time on the holistic assessment and review of complex cases to inform the development of a comprehensive care plan and, wherever possible using available clinical time to create opportunities to supervise, teach and assess junior staff in practice, developing their competency in identifying subtle changes in condition and knowing when to escalate within the team or to refer to specialist review.
Day to day ensure that the allocation of work is appropriate to staffing levels, skills and competence of available staff and facilitate the prioritising of referrals into the service ensuring that urgent referrals are responded to within an appropriate timescale and feedback is provided if appropriate to the referring health professional.
Triage referrals coming into the team, ensuring an appropriate response, and liaising with referrers where required.
Oversee duty systems within the team to enable same day response to urgent referrals where needed, working closely with Rapid Response, Social Care, and the Urgent Response Therapist to ensure seamless pathways are in place for patients in crisis.
