Job summary
We are currently looking for a Diabetes Specialist Nurse to work within the Community Diabetes Service based in Portsmouth.
In this role you will be responsible for providing specialist clinical knowledge and skills and support to people living with diabetes, their families and carers in a variety of Community Settings. You will provide timely specialist clinical advice and education to health care professionals in Primary Care via email, telephone or MDT joint reviews/clinics. You will facilitate education programmes for patients, family members and healthcare professionals.
We are looking for someone that has experience in working in a team, but also able to work as an independent practitioner, maintaining a high professional standard of care.
You need to be able to demonstrate good written and verbal communication, be organised, flexible, and approachable with the ability to deal with day to day challenges. The successful applicant should demonstrate excellent time management skills, be self-motivated with a positive attitude.

Main duties of the job
The Community Diabetes Nursing Service work as an advisory service, Offering education, training and support to primary care and community services clinicians. This enables primary care/community services to manage adults with diabetes as effectively as possible.
Clinical Responsibilities
Responsible and participate in MDT Clinics and group education for the purpose of enabling and empowering individuals to maximise their health potential.
Provides clinical leadership, advice, guidance as part of the DSN team and other health professionals within the primary, community and secondary care setting, including medical and nursing teams, the voluntary sector and other interested agencies.

Works across traditional boundaries promoting the development of integrated and effective links between the primary, community, and secondary care interface so that people with diabetes receive care and education appropriate to their needs.
Provide timely advice, support and information to health care professionals and people with diabetes by email and telephone.
