Job summary
We are looking for a dedicated Mental Health or Learning Disability Nurse to join us on our Kite Unit at the South of England Rehabilitation Centre (SERC).
The Kite unit is a ten-bed neuropsychiatric rehab service for adults following an acquired brain injury, whose impairments are largely in the cognitive, behavioural, or mental health spectrum, with the aim to enable patients to rebuild their lives after a brain injury. We are proud to have been accepted on the pathway towards accreditation with the Quality Network for Neuropsychiatry Services.
You will be responsible for meeting the needs of patients and their families to achieve improved health outcomes and promote health and wellbeing while demonstrating care, compassion, competence, communication, courage, and commitment. We are passionate about patient centred, culturally sensitive and evidence-based care which promotes independence.

Main duties of the job
- Assess patients, plan, develop, implement and evaluate programmes of care and carry out nursing procedures.
- Provides direct nursing care for patient to achieve best health outcomes.
- Ensures use of nursing knowledge and skills to assess, plan, implement and evaluate patients/clients/family/ health needs as well as actively promote health and wellbeing.
- Contributes to effective discharge planning and referral processes
- Manage care of patients on a day to day basis and when required manage care of all patients in the absence of senior colleagues.
- Accurately document assessments of patients health status, diagnosis and response to nursing and other interventions and treatments on the Trust Clinical Records System.
- Educate and provides information to patients to improve their recovery.
- Role models culturally safe nursing practice and report poor practice when observed.
- Practices in accordance with legal, ethical, culturally safe, and professional standards including the Code of practice.
- Acts as a named nurse/key worker.
- Plan and co-ordinates complex healthcare packages that are responsive to the individuals changing healthcare needs i.e. challenging behaviour, mental health and complex physical health needs.
- Being a Student Assessor, a Shift Co-ordinator as well as supervising other staff.
- Accepting, reviewing and auditing Mental Health paperwork
- Checking rotas and sending unfilled shifts to Bank in the absence of the Ward Manager
- Attending meetings in the absence of the Ward Manager
