Community Occupational Therapist

Job summary

An opportunity has arisen for an Occupational Therapist to join our Community Occupational Therapy team located in Portsmouth.

We are integrated with the Health and Social Care Team and co-located with Social Workers and work closely with Community Nurses and Physiotherapists. We work across Portsmouth assisting people 18 years and over with physical, mental and social impairment to optimise their functional independence and quality of life.

In this role you will:

  • Work with disabled people and their carers in their own homes, assessing needs and implementing agreed action plans.
  • Assess and advise on adapting methods of carrying out daily activities to support individuals in managing their disability and promoting independent living and Wellbeing.
  • Use assistive technologies and design and construction methods to improve, restore and attain occupational performance.
  • Have the opportunity to develop expertise in seating and posture management, moving and handling and assistive technologies.
  • Recommend, and provide where appropriate, specialised equipment and demonstrate and give guidance to clients and carers in the use of equipment.
  • Provide support and advice to clients and their families/carers in their adjustment to disability and long term conditions.

You should be a car driver and access to suitable transport is essential.

In return you will receive Clinical and Management Supervision including regular Personal Development Plans and other development opportunities.

Main duties of the job

Manage a caseload of patients providing interventions using evidence based, patient centred principles.

Ensure the service provided is high quality, clinically effective and cost effective. The post holder has responsibility for a caseload of patients, assessing need, formulating intervention plans, delivering interventions and evaluating outcomes.

Working under the supervision of a Senior OT you must ensure that you work within the scope of your competency and that your practice meets Trust and professional standards.

Our aim is to assist people of all ages with physical, mental and social impairment to optimise their functional independence and quality of life.

You will be assessing, advising on and adapting methods of carrying out daily activities to support individuals in managing their disability and promoting independent living and wellbeing.

Use assistive technologies and design and construction methods to improve, restore and attain occupational performance.

In addition, you will be encouraged and supported to develop expertise in seating and posture management, moving and handling, assistive technologies and Independent Mobility Assessments as part of the Blue Badge assessment contract that we have. You will recommend, and provide where appropriate, specialised equipment and demonstrate and give guidance to clients and carers in the use of equipment.

Provide support and advice to clients and their families/carers in their adjustment to disability and long term conditions.
