Job summary
We are looking for a Care Coordinator to join our team!
Solent GP Surgery is based across 3 sites- Portswood, Adelaide and the Royal South Hants, holding a patient list of approximately 20,000
The Care Coordinator will be a key point of contact for a vulnerable cohort of homeless patients with mental health, substance abuse and alcohol issues and long-term conditions, and within the practice team will help people to meet their needs, answer their queries and provide coordination and support for them.
The key focus for the role is to lead supporting patients personalised care requirements. The Care Coordinator will work closely with the practices and PCN teams to manage an identified caseload of patients, including ensuring all the patients registered with the practice who require LTC review and monitoring are on the relevant register, that their review appointments are in place, and working with the practice team and patients to ensure any follow up arrangements are in place.
As part of the role, you will work closely with all members of the practice and PCN team including the administrative team, nursing team, mental health team and PCN Personalised Care team.
To work without direct supervision and show extensive use of initiative in prioritising and making decision in relation to effectively supporting the smooth operation of the surgery.
Main duties of the job
Assist patients to access self-management education courses, peer support or interventions that support them to take more control of their health and wellbeing in relation to the PCN Service Specifications.
Provide co-ordination and navigation for patients and their carers across health and care services, working closely with social prescribing link workers, health and wellbeing coaches, and other primary care where relevant.
Work with the GPs and other primary care professionals within the PCN to identify and manage a caseload of patients, and where required and as appropriate, refer people back to other health professionals within the PCN which could involve the exchange of complex information.
Support weekly mental health multidisciplinary team meetings and monthly team MDT and other events within the organisation as mutually agreed, producing comprehensive and accurate minutes.
Support patients to manage their vaccinations through answering queries, making and managing appointments and ensuring that they have good quality written or verbal information.
To support the Practice to meet targets, including areas identified by their practice manager such as QoF projects and KPIs.
Signpost patients and their carers to appropriate support both inside and outside the practice.
Book appointments accurately for call/recall ensuring sufficient information is recorded.
Ensure appropriate appointments have been made in accordance with the patient's annual review.
