Job summary
We are looking to recruit a part time Healthcare Support Worker into our Andover and Winchester Rural North Community Team. As a valued member of our community nursing team, the role will involve providing high quality care to our patients in their own homes
Work will be delegated by a registered nurse and will include all aspects of healthcare delivery that are appropriate for your scope of practice.
There is a requirement to demonstrate excellent communication skills; using these skills to engage with patients who often may exhibit complex and at times challenging behaviours and needs, to ensure the most appropriate care and support is provided.
There is an expectation that the role will adhere to local and organisational policies and procedures when undertaking your work.
Appropriate training will be provided to ensure you are proficient and competent to undertake this role.
Main duties of the job
To assist qualified staff in the provision of community-based care delivery and undertake a variety of tasks to facilitate the smooth running of the service as directed by the team and service lead.
Clinical tasks may include but not limited to:
Undertake phlebotomy for patients in their own homes.
Provide diabetic care to patients including blood glucose monitoring and insulin administration.
Undertake wound and leg ulcer care
Catheter care and re-catheterisation
Undertake basic observations such as blood pressure measurement, record appropriately and feedback to team lead and /or GP
Obtain and process a variety specimens appropriately
Support and liaise with doctors and other members of the MDT
Report any changes in condition of the patient to the District Nurse / Team Lead or in their absence the designated triage nurse in charge.
Develop rapport with patients and treat all patients, carers and staff with dignity and respect
