CRS is a multi-disciplinary team made from staff employed by Health and Adult Social Care for residents of the Portsmouth City area. The service aim is to help individuals remain at home and to avoid unnecessary admission to hospital, and to facilitate discharge from hospital by offering assessment of immediate needs.

The team work hard to ensure high quality person-centred care is at the heart of everything we do, and our staff are supported in their roles and encouraged to develop. You will be required to work within the wider multi-disciplinary team providing evaluating and communicating effectively with other teams within the Service.

Main duties of the job

You will work as a member of a multidisciplinary team alongside service users to promote independence and daily living skills, which will include supporting with personal hygiene needs, diet, fluids and meal preparation, encouraging them to engage in social or leisure activities as identified within their Care Plan.

This role will provide support in management of risk, developing therapeutic relationships, supporting people to live meaningful and fulfilling lives.

Act as a role model for team members, supporting new or less experienced colleagues.

This role will assist and support qualified staff and carry out assigned tasks as directed, without supervision. This will include housekeeping duties where required.

This role will demonstrate excellent communication skills, using them to engage, persuade or negotiate with clients with complex, at times challenging needs to ensure they are provided with the most appropriate care and support.

Undertake clinical and environmental risk assessments and document key risk areas, seeking guidance from more senior staff when dealing with cases indicating a high level of risk.

Accurately assess, describe, and record observations on progress or change in both patients' mental and physical state including clinical observations-BP, pulse, respiratory rate, pulse oximetry, temperature and blood glucose levels using appropriate assessment tools, escalating any concerns as appropriate.

  • Healthcare qualification Level 2, 3 or working towards gaining equivalent level.
  • A - C 9 -- 4 GCSE in Maths and English or equivalent.

Experience
