Job summary
Are you interested in becoming part of an innovative community Older Persons Mental Health Service, which focuses on all areas of mental health, irrespective of functional, organic or complex illness on the Isle of Wight?
We are aligning our focused OPMH pathways with all age Community Mental Health Services and are looking for passionate, highly motivated individuals to work as part of the Older Persons Mental Health Service. The post holders will undertake the role of care co-ordinator / care manager in the assessment, treatment and delivery of care to older people with mental health problems who meet the criteria for access to the service. They will be responsible for ensuring that the highest quality care is carried out in accordance with policies and procedures Hampshire and Isle of Wight NHS Foundation Trust.
You will need be a Registered Mental Health Nurse (RMN) and have experience of working with people with serious and enduring mental health needs. The ability to work in partnership with carers/family members and services in the community is essential.
Main duties of the job
