Senior Medicines Management Pharmacy Technician

Job summary

We are seeking a Senior Medicines Management Pharmacy Technician to lead the Hampshire and Isle of Wight Healthcare NHS Foundation Trust's Medicines Management Pharmacy Technician provision to services on the Isle of Wight.

The post holder will

*provide training on Medicines Management processes to other pharmacy staff and the wider MDT as required

*analyse selected audit data and produce audit reports for use by the Pharmacy and Medicines Management teams and the wider Trust

*organise daily operational process within HIOWH services on the Isle of Wight

The work carries a strong clinical component and will involve providing patient counselling, checking drug histories, and updating electronic medication records, assessing the patient's ability to self-medicate and providing a POD verification service on the wards at a variety of locations.

Main duties of the job

The successful applicant will possess sufficient knowledge and experience such that they are able to work autonomously, relay sensitive information with tact and care, and:

*Assist in the medication reconciliation process confirming medication currently taken by patient either verbally from patient, relative, GP, or by consulting medical notes and record this in the EPMA pharmacy system.

*Appropriately complete all necessary documentation in the units visited.

*Ensure medication is transferred with the patient to avoid duplication of supply.

*Complete all relevant electronic forms in the event of an error being found on the ward inform and inform the ward pharmacist if errors have been found.

*Ensure appropriate patients receive a Medication reminder card on discharge.

*Assess suitability, quality, and identity of PODs for extended use. Dispose of any discontinued or unsuitable patients' own medicines according to local procedure and guidance.

*Assess suitability of patients' own drugs for discharge medicines and organise a timely supply of new or additional items using the EPMA system and according to local procedures and guidelines.

*Provide medicines management support to other community services areas as agreed by the line manager and cover other team member's leave and sickness across all sites the medicines management team provides support.

Job description

Job responsibilities

Dispensary based role.

* Undertake sessions within a dispensary to maintain ACT competency (two hours per week).

* Use the pharmacy EPMA system to enter patient details to supply medication and produce a dispensing label, annotate JAC produced orders and prescriptions as set out in guidelines, and be able to access various other programmes for information of previous history.

* Arrange prompt dispensing and delivery of non-stock supplies for patients in line with the hospital formulary, guidance on restricted medication and according to procedures.

Administration

* To assist and supervise stock control issues if identified and review stock lists with the unit pharmacist and nurse in charge.

* To collect data for any projects and audits identified as being required by the Trust or their commissioners. To assist in the production of reports from the data collected.

* To support the use of clinical systems such as EPMA and stock control systems. This will include supporting users to use the system safely and support in times of business continuity.

* To provide input into improvement projects to make sure the EPMA system and associated pathways meet the needs of the service.

Communication and Analytical / Judgement

* To liaise with GP practices, community pharmacies and hospital teams over medication related issues to ensure the patient has prompt access to medicines and a smooth transition from secondary to primary care when this occurs.

* To assess compliance with medication and the patient's ability to self-medicate. Where necessary to organise compliance aids for individual patients (Blister packs, Dosett boxes, MAR charts etc) to support independence.

* To provide information and counselling on the use of medicines to selected patients/carers and other health care professionals taking into consideration the level of understanding and emotional well-being.

* To independently resolve discrepancies and make interventions involving patient's medication with the prescriber whenever possible.

* To refer to the ward pharmacist or appropriate health care professional if necessary and document any intervention on the clinical record.

Planning & Organisation

* To participate in the delivery of education and training sessions to the multidisciplinary team, and patient groups undertaken by the pharmacy team in line with the relevant Trust's Medicines Policy and to patient education groups.

* To plan and deliver Medicines Management Training for Pre-registration Pharmacy Technicians, in co-ordination with the pharmacy training lead.

* To plan and deliver Medicines Management training for Medicines Management Technicians within HIOWH and IOWT as appropriate

* Undertake training and mentoring of staff and other groups as required.

* To take responsibility for identifying own training and development needs, suggestions for improvements to the service and policy changes to the line manager.

* Ensure compliance with Trust arrangements for continuing professional development (CPD) and compliance with the General Pharmaceutical arrangements for revalidation and CPD.

The post holder will work with considerable professional autonomy in their assigned areas of responsibility, while escalating complex issues to their line manager as appropriate. They will:

Make day-to-day decisions about medicines supply and medicines safety within their scope of responsibility

Manage their own workload

Respond to and resolve routine operational and medicines-related queries

Contribute to the development of local medicines policies and procedures

Take immediate action when required to maintain safety, escalating significant issues appropriately

* To deputise for Chief Pharmacy Technician as appropriate in partnership with peer colleagues and represent the team at meetings.

* To manage the team rota on a day-to-day basis and where necessary provide annual leave and sickness cover to other sites within the Trust.
