Job summary

Due to the exciting transformation of the Memory Assessment Service we are currently seeking highly skilled, motivated, passionate practitioners with a desire to provide high quality, timely care.

You may already be a B6 or looking for your next step. We are keen to hear from you with the desire to work with individuals referred into the memory assessment service.

Main duties of the job

To provide thorough memory assessments for those referred into the service

To gather a detailed collaborative history of presenting problem from the patient and their carer

To gather relevant past medical history from system partners

To be able to undertake physical health observations and awareness of escalation routes

To formulate plan of treatment in collaboration with senior colleague in MDT.

To assess risk and develop management plans, reviews and adjust in line with the service user's needs.

To consider the holistic care needs and identify when and how to appropriately refer on to services that can meet the individual needs

To work in collaboration with service users & their family/carers as experts in their own care

  • Knowledge/understanding of dementia and associated behaviours/interventions

  • Knowledge of the key principles of the Mental Capacity Act
  • Knowledge/awareness of the current NICE guidance relating to dementia and memory assessment pathway
