Job summary

The post holder has responsibility to provide a physiotherapy within a designated team.

The assessment and interventions undertaken will be embedded within the framework of:

*MDT Principles of working.

*The holistic model of care (Four Tier Model).

*The seven areas of clinical practice.

Work as part of the specialist multidisciplinary team delivering services to people with a learning disability in the community to deliver specialist physiotherapy assessment and intervention for people with learning disability, and additional complex physical disability, mental health problems or behaviours of challenge.

Responsible for a designated caseload including, assessment, treatment planning, intervention and advice, discharge planning and record keeping.

Carrying out home visits within the community as a lone practitioner with telephone support or accompanied by other members of the MDT as appropriate.

Identifying appropriate settings for treatment, facilitate access and provide support to service users, families and support workers to enable them to continue treatment.

Teach carers/ support workers physiotherapy treatment programmes.

Utilise resources and equipment appropriately; Joint Equipment Store, Wheelchair services etc.

Fit and operate standard equipment e.g. standing frames etc according to instructions and report defects appropriately.

Undertake routine tasks to support the department e.g. collect statistical information about daily service users contacts, take part in Audits etc.

Main duties of the job

*Deliver and coordinate the physiotherapy clinical services to people with learning disabilities whose health needs fit within the 7 clinical areas of practice, which are mental illness, complex physical health, epilepsy, challenging behaviour, forensic, autism and dementia; working in a multi-disciplinary team and to participate in specialist multi-disciplinary/joint assessments of clients as required, in line with team process.

*Undertake detailed assessment for people with complex physical disability and provide appropriate management programmes, with the support from the Band 7 physiotherapist; embedding learning in care planning and therapeutic practise including aquatic therapy and advice to achieve high quality outcomes for service users.

*Teach carers and support workers physiotherapy treatment programmes and address individual moving & handling needs.

*Work within the LD Service's Single Point of Access on a rotational basis undertaking clinical assessments to assess the eligibility and health need of individuals referred into the service.

*To use the most appropriate communication methods when working with service users who have communication difficulties to build rapport and enable the service users to have choice and influence over their intervention, communicating effectively and sensitively with service users and carers with respect to their views and culture.
