We are looking for a Clinical Specialist/Team Lead Therapist to join our Community Rehabilitation Therapy Service on the Isle of Wight.
The Community Rehabilitation Therapy Service is commissioned to provide a 7-day Rehab Service. You will be the lead Therapist working in or Community Rehabilitation bedded care workstreams and may include other settings such as Domiciliary visits, Rehabilitation Out-patients as well as UCR pathways. The role provides a high standard of Therapy across all settings. You will be required to always act in the best interest of the patients under your care.
You will work autonomously as a Clinical Specialist Therapist to provide advanced assessment, clinical diagnosis and treatment of patients requiring rehabilitation, who present with a wide variety of clinical conditions that may be complex including long term conditions, neurological, orthopaedic, medical, surgical, falls and palliative diagnoses.
To take responsibility for the leadership and overall development of the Community Rehab Bedded Care team, especially regarding all aspects of clinical governance, ensuring that the service maintains high standards of care that are effective and sustainable, alongside other Clinical Specialists, to ensure best practice is embedded in the setting.
To act as an independent source of expertise and specialist advice on rehabilitation across the Trust and wider healthcare community including senior therapists. other health care professionals, GP's, and Consultants.
Main duties of the job
To provide rehabilitation/ AHP specialist clinical leadership to members of the Community Rehabilitation Therapy Team in conjunction with the Service Lead.
To take a lead Therapy role in the advanced assessment and treatment of patients who will frequently have complex and/or chronic presentations as required, both in general and neurological rehabilitation assessment
To autonomously determine clinical diagnosis and develop effective treatment plans and mentor team clinical practice.
To manage your own, and the team's caseload ensuring commissioned rehabilitation beds specifications are met and the commissioned services are provided.
To keep up to date with developments in the rehabilitation specialist fields working closely with other clinical specialists to maintain and progress advanced clinical assessment and treatment skills within the team as part of the AHP specialism in rehabilitation.
To independently supervise the Community Rehabilitation Therapy Bedded Care Team, initiate and participate in evidence-based quality improvement projects to audit and evaluate practice and when required, be responsible for implementing changes within the Rehab, Recovery and Unplanned Care pathways alongside Locality pathways.
To formulate individualised management and treatment plans, and utilising highly developed treatment skills, recommend the best course of intervention leading to a comprehensive specialist programme and a timely discharge; mentoring the team to deliver quality clinical practices
