Job summary

Are you interested in being at the forefront of transforming clinical pharmacy? We seek a highly motivated and enthusiastic pharmacist to join our expanding team. Building on the success of our primary care pharmacy programme, we are excited to offer a position that combine roles in GP practices with responsibilities in hospital wards and departments. Future opportunities to explore sector-specific roles will be available. The position is patient-centred, enhancing your clinical and technical skills in practice.

You will receive support from an educational supervisor and the wider pharmacy team, along with peer support from fellow pharmacists. Successful applicants will be guided in completing the RPS Post Registration Foundation portfolio. This course includes the independent prescribing qualification.

Our team operates across community hospital sites, general practices, mental health services, and specialist teams. Partnership working is central to our values. We collaborate closely with other members of the multi-disciplinary team, including our community pharmacy colleagues, to deliver the highest quality care in Hampshire. Opportunities to participate in various projects within the integrated pharmacy service and help shape the service's future are available. Experience in a hospital or primary care setting is not essential, as comprehensive training and support will be provided. Positions are also open to pharmacists currently undertaking their postgraduate foundation training year.

Main duties of the job

The post holder is a pharmacist who acts within their professional boundaries, working alongside a team of pharmacists in general practice and within the Primary Care Network (PCN) and medicines management team as part of Hampshire and Isle of Wight Healthcare (HIOWH) community and mental health NHS Foundation Trust.

The role will be delivered in the primary and secondary care setting, specifically at general practice with the PCN and Western Community hospital wards. The clinical practice will focus on areas such as medication management, chronic disease management, and patient safety, emphasising a holistic approach to pharmaceutical care.

This role will facilitate collaboration with various healthcare professionals, including general practitioners, nurses, and specialists within primary and secondary care. The pharmacist will be a vital link in the multidisciplinary team, ensuring effective communication regarding patient medication histories and optimising treatment plans. Regular interactions with hospitals will enhance the flow of information on patient medicines, thereby improving continuity of care and patient safety.

The anticipated outcomes of this role include improved health outcomes for patients through the safe and effective management of their medications, enhanced efficiency in the primary care team, and greater patient satisfaction.
