An exciting opportunity has arisen within Southern Health Foundation Trust to appoint an inspirational and enthusiastic Health Care Support Worker to join the Hayling Community Care Team, based at Havant Health Centre. The team covers 3 GP practices in Emsworth and Hayling Island with a practice population of 34,000.

The Post is fixed term for 4 years to 2029 covering a HCSW who is currently at Open University completing RGN Training.
We are looking for a motivated professional with excellent communication skills who has the ability to work independently and as part of a busy team, to deliver a high standard of nursing care across community health care settings.
The post holder must have relevant experience in health/social care and demonstrate a proactive attitude to learning the skills required to fulfil the role.
Access to a vehicle and possession of a full driving license is essential.

Main duties of the job
The post holder will have responsibility for participating in the delivery of patient care under the supervision of the Community Care Team.
*Work under supervision following written care plans and care pathways devised by the Registered Nurse.
*Follow prescribed care plans and treatment protocols.
*Follow HPCT Policies and Procedures.
*Has knowledge and skills to carry out prescribed tasks and will report to Registered Nurse any variances.
*Delegated responsibility for Patient Support Teams administrative tasks.
*Provide general administration support to the team.
