Job summary
Are you an Occupational therapist wanting to work within acute community therapy in Mid Hampshire? The division is passionate to provide patient centred care and rehabilitation within the population's own environment. Teams are responsive, well led and caring providing guidance to patients, relatives, carers and staff.

UCR is a dynamic team delivering acute level treatment and intervention to people at risk of hospital admission in their own homes. The Occupational Therapists play a pivotal role in preventing deconditioning, keeping people at home and enhancing their wellbeing. This role includes working on the collaborative falls car alongside South Central ambulance Service. The team comprises a wide range of professions delivering complex and comprehensive care to frail and vulnerable patients at home. The therapy team within UCR is developing and growing, with some exciting changes planned. The service runs 7 days a week, 365 days a year.
Main duties of the job
To work closely with the Consultant Practitioners, Advanced Clinical Practitioners (ACPs), the Allied Health Professional Lead, Team Lead and the wider nursing and therapy staff, as well as GPs to provide fast reactive services for patients with exacerbation of long term conditions and/or frailty, ensure rapid delivery of treatment, care planning and admission avoidance where appropriate.
The occupational therapist will work across the caseload and the single point of access, using their clinical skills to identify the needs of patients and liaise with the correct services.
The occupational therapist will provide expertise in their own discipline to the wider team.
Promote health and prevention of illness and disease by providing information to individuals where possible. Recognise situations that may be detrimental to health for example housing, social and economic factors and liaise with appropriate team members.
To provide complex case management using extended skills where appropriately trained to avoid hospital admission and manage complex clinical needs in the community setting.
