Job summary

Are you a Children's Nurse passionate that no child should be in hospital if care can be provided closer to home? Do you want to build on your clinical skills in the community?

Join our evolving Children's Hospital @ Home team! In collaboration with University Hospital Southampton NHS trust (UHS), the service works closely with the medical team at the hospital and GPs in the community to facilitate acute assessment with the aim to prevent & minimise admissions to hospital and facilitate early discharge where possible.

There's lots of training available to gain new and enhance existing skills and will be development opportunities available with future plans for ANPs within the team.

Main duties of the job

The Children's Hospital @ Home (CH@H) team work closely and seamlessly with partners in acute trusts and primary care networks as part of an integrated health offer for children and young people.

The Children's Hospital @ Home team provides specialist nursing care, support and co-ordination of care; predominantly to children and young people with short-term acute health care needs requiring urgent care pathways. Our care is led by Wessex Healthier Together. As a service we work closely with University Hospital Southampton. CH@H delivers service promotion to all referrers across the region including GP Surgeries, Children's Emergency Department, Paediatric Short Stay Unit and Paediatric Wards. Support is provided to children and families via telephone calls, video calls and home visits. The service aims to provide high quality nursing care which enables early, safe planned discharge from hospital and prevents unnecessary admissions.

With the support of the Senior Nursing Colleagues and the Clinical Team Co-ordinator, you will act as a resource of paediatric expertise, providing specialist advice to other professionals, promoting evidence-based practice.
