Are you a Children's Nurse passionate that no child should be in hospital if care can be provided closer to home? Do you want to build on your clinical skills in the community?

Join our evolving Children's Hospital @ Home team! In collaboration with University Hospital Southampton NHS trust (UHS), the service works closely with the medical team at the hospital and GPs in the community to facilitate acute assessment with the aim to prevent & minimise admissions to hospital and facilitate early discharge where possible. There's lots of training available to gain new and enhance existing skills.

Main duties of the job

Provide specialised, high quality skilled nursing care, considering the physical, psychological, spiritual and social needs of the child with nursing care needs.

Support the management of the integrated acute and community care pathways for children with short term conditions

Provide supervision and monitoring of junior staff with support of clinical team leader. Performing individual performance reviews for junior team members.

Work with families to provide emotional support and specialist clinical care, sensitive to the changing and on-going needs of the child, siblings, parents and other carers.

Maintain regular liaison with local paediatric unit, paediatric emergency department, primary health care team and other agencies to ensure good communication and continuity of care for the child

Working collaboratively with all professionals to enable children/young people to be cared for at home whenever possible. The information received is often of a complex and sensitive nature requiring sensitivity, empathy and reassurance.

Demonstrate excellence in clinical practice, keeping up-to-date with new developments in nursing care and treatments ensuring practice is evidence based by utilising current research.

To actively participate in and promote Clinical Supervision and Reflective Practice.

Maintain a personal and professional profile and fulfil training requirements as required by the Nurses and Midwives Council (NMC)
