Job summary
There is an exciting opportunity for successful applicant to be part of a dynamic multidisciplinary Community Mental Health Team working in the Intensive Case Management Team.
The Intensive Case Management Team is an integral part of the Community Mental Health Team and the team provide high quality holistic support for service users including individuals with complex needs.
The post holder will be expected to work with individuals who are experiencing a variety of mental health issues to promote recovery, reduce risks, assist on positive well being and functioning within the community.
While doing the job, the post holder will actively participate in the assessment of needs following care plan process caring for service users and ensuring care is collaborative and individualised.
The successful applicant will join a friendly , passionate ,supportive and caring team working in an environment that motivate and promote individual development.
Main duties of the job
To work as a member of the team providing mental health services as required to patients in the Intensive Case Management Team case load, and Community Mental Health Teams following assessment
To be available to work flexibly across 5 day a week service.
To assist in the delivery of high standards of care and support to individual clients working collaboratively with qualified nurses, social workers, occupational therapists, Psychologist and Doctors.
To offer intensive support to people under Intensive Case Management Team (ICM) working with people with complex needs and difficult to engage individuals
Engage with Team members in using Assertive Outreach Approach to support required service users to meet identified needs.
