Job summary

There is an exciting opportunity for successful applicant to be part of a dynamic multidisciplinary Community Mental Health Team working in the Intensive Case Management Team.

The Intensive Case Management Team is an integral part of the Community Mental Health Team and the team provide high quality holistic support for service users including individuals with complex needs.

The post holder will be expected to work with individuals who are experiencing a variety of mental health issues to promote recovery, reduce risks, assist on positive well being and functioning within the community.

While doing the job, the post holder will actively participate in the assessment of needs following care plan process caring for service users and ensuring care is collaborative and individualised.

The successful applicant will join a friendly , passionate ,supportive and caring team working in an environment that motivate and promote individual development.

Main duties of the job

To work as a member of the team providing mental health services as required to patients in the Intensive Case Management Team case load, and Community Mental Health Teams following assessment

To be available to work flexibly across 5 day a week service.

To assist in the delivery of high standards of care and support to individual clients working collaboratively with qualified nurses, social workers, occupational therapists, Psychologist and Doctors.

To offer intensive support to people under Intensive Case Management Team (ICM) working with people with complex needs and difficult to engage individuals

Engage with Team members in using Assertive Outreach Approach to support required service users to meet identified needs.

Qualifications

Essential

  • NVQ 3 or Health Related Qualification or equivalent experience in an Acute/Crisis mental health setting/Rehabilitation setting.

Experience

Essential

  • Experience with working with people and patients, carers.
  • Experience of working within a multidisciplinary team.

  • Computer literate.
  • Communication and relationship skills.
  • Literacy skills.
  • Interpersonal skills.
  • Good time keeping, planning and being organised.
  • Admin/clerical skills.
  • Willingness to work flexibly.
  • Ability to de-escalate and use Breakaway techniques.
  • To be diplomatic and to possess conflict resolution skills.
  • Occasional exposure to highly unpleasant working conditions e.g. during home visits and or during assessment.
  • Ability to problem solve.
  • Knowledge of mental health diagnosis and appropriate care.
  • To have a working knowledge of Risk Assessment and management tools currently in used within AMH.
  • An understanding of the principles of equal opportunities as it relates to staff and patients and able to demonstrate personal commitment to challenging discrimination and promoting equalities.
  • The role requires Enhanced DBS.
  • The role requires the post holder to sign up to the Automatic Renewal DBS service.
  • Is able to work legally in the UK.
