Job summary
This is an exciting opportunity to join our Vocational Rehabilitation Service, providing 'return to work' education, advice, rehabilitation and support to patients with a Neurological diagnosis.
This role would be best suited to an individual with experience working within health or social care settings, who has experience working with people and who possesses good communication and reasoning skills. The ideal candidate will be confident working face to face and virtually.
This post requires an NVQ level 3 or equivalent experience relevant to health and social care. Existing coaching skills and awareness of both the physical and mental health impacts of a neurological diagnosis are also desirable.
Solent Vocational Rehabilitation Service is part of the wider Community Neurological Services and is available to Hampshire and IOW residents. Service delivery includes assessment, digital innovation application, and group work. The service is multidisciplinary crossing primary, secondary and community care. The service core clinical staffing of community physiotherapy, occupational therapy, nursing and non-clinical care co-ordinators with weekly multi service and profession MDT's for complex patient care in parallel or signposting. Rehabilitation Assistants for Vocational Rehabilitation Service support patient to navigate the complex pathways and support in the treatment pathway.
Main duties of the job
The purpose of this position is to provide Senior Vocational Rehabilitation Assistant intervention to a specified caseload of patients with Stroke and Neurological Diagnosis as part of a specialist team, delivering neurorehabilitation and vocational-rehabilitation programmes based upon individualised patient goals.
The Vocational Rehabilitation Service is an Occupational Therapy led service dedicated to working with people with a variety of neurological conditions including Stroke, Brain Injury, MS, Parkinson's and Spinal Injury. Vocational Rehabilitation intervention aims to support an individual to achieve goals to succeed in a vocation and return to paid employment, unpaid work experience, voluntary work and, where appropriate, meaningful occupation.
Rehabilitation can be delivered in a variety of settings and is tailored to meet the needs of the individual, with the outpatient setting offering facilities that include a workshop, kitchen, IT suite and garden. Streams of rehabilitation include a group workshop to complete individual project work, the 'Building Skills for Work' group programme and 1:1 Occupational Therapy sessions. The Vocational Rehabilitation Service works in close collaboration with the wider Solent Neuro Rehabilitation Services, will liaise directly with employers and legal departments and has a working relationship with voluntary sector providers supporting access to employment and individuals with Neurological diagnosis.
