If the answer is yes, a new role within the New Milton and Lymington therapy team may be of interest.

We are looking for a Band 6 Therapist to join our very friendly and supportive team of Occupational Therapists, Physiotherapists, Therapy Clinicians, and Rehabilitation Assistants who work with people in their homes.

The team's role spans from Urgent Community Response which aims to keep people at home in times of sudden change in their mobility/ability to rehabilitation intervention after illness/change in health condition where people are empowered to reach their best potential.

We are based with the District Nurses, Older People Mental Health and the Frailty Support team in an office on the road between New Milton and Lymington and cover a beautiful area from the coast from Barton-On-Sea to Lymington, and inland through the New Forest to Brockenhurst and East Boldre.

Applicants must be a UK Health Professions Council registered Occupational Therapist/Physiotherapist OR a Degree in Sports and Exercise Sciences / Sports Rehab or appropriate related field and be registered with the Society of Sports Therapists or BASRaT, and in possession of a full driving license and vehicle to be used for work. The post holder will also need to be willing to work flexibly to support our 7 day service model.

Main duties of the job

-To work as part of the community therapy team and take the lead in the Urgent Community Response for the team.

-To assess people at home who are at risk of hospital admission in next 24-48 hours due to a sudden and acute change in health condition such as infection or progression of palliative disease which results in sudden change in mobility. (N.B Patients may not have already had a medical assessment before seeing the therapist and therefore may need the clinician to recognise a deteriorating patient presentation and refer on for medical assessment when required).

-To establish a safe way to remain home if possible, or if not able to refer on to appropriate location to support their needs urgently to reduce likelihood of further injury/deterioration.

-Urgent Moving and handling assessment.

-Urgent positioning/pressure management.

-Urgent Falls interventions.

Other tasks:

-Referring on to other professionals within One team (District Nurses, Frailty Support Team), as well as GP, Charities, social services, and acute hospitals.

-Ordering equipment.

-Triaging new referrals to therapy team.

-Providing rehabilitation where capacity allows.

Essential

  • - UK Health Professions Council registered Occupational Therapist/Physiotherapist
  • OR
  • - Degree in Sports Therapy, Sports Rehab or appropriate related field and be registered with an externally regulated body such as Health Care Professionals Council or BASRaT (we accept registration with the Society of Sports Therapists (SST).
  • - Evidence of community experience and working as part of a multidisciplinary team.
  • - Current (UK/EU) Driving Licence

Desirable

  • Recently updated appropriate post-graduate course attendance.

Experience

Essential

  • - Experience of working within a variety of clinical settings.
  • - Evidence of attendance at appropriate study days, short courses or evidence of undertaking self study

  • - Experience of supervising assistants or students
