Clinical Care Navigator No Wrong Door

Job summary

Across Hampshire and Isle of Wight (HIOW) we have launched our plan to transform experiences in adult mental health in a programme called 'No Wrong Door'. This is a new and innovative service led by the local NHS, councils, Public Health, GPs, voluntary and community partners and people with lived experience of mental illness.

Working across several PCN's you will play a highly supportive role in joining both clinical and non-clinical pathways for patients.

The day-to-day role varies and can include:

*Support with overcoming challenges e.g. getting on a bus

*Signposting to different organisations for support

*Researching different organisations offering support e.g. clubs, social interaction etc.

*Skills sessions with patients e.g. Decider skills. Full training will be given.

*Managing own caseload and diary

*Working with different organisations

This role would suit someone with the following skills set:

*Ability to work independently

*Good organisational skills

*Proactive approach

*Would suit someone who has had community Health Care Support Worker experience

The person needs a full driving licence or the ability to travel to location in the area(s) they are based. The locations available are; Havant, Petersfield, Portchester and Fareham

The role would be based within these areas, with requirements to work across a variety of sites including Southern Health. Primary Care Networks (PCNs) and other organisations e.g. Job Centres and Citizen Advice Bureau (CAB).

Main duties of the job

To work across a range of clinical and non-clinical services to support the delivery of transforming mental health care.

To support the planning, co-ordination and transfer of service users across the care pathways.

To support clinicians to identify care needs

To support the services in assisting patients through the clinical and non-clinical pathways.

To assist and identify areas where barriers to health and social care exist and be part of the team working to resolve this.

To work in a manner which is helpful to the service user, based on principles of recovery, personalisation and choice.

To develop the role of Care Navigator in order to embed the No Wrong Door approach within the system.

To work with stakeholders and care teams to enable access to relevant interventions / support and self-management as part of the Care Navigator role.

The final combination of Southern Health and Solent NHS Trust services is expected in late 2024, when our new organisation, to be known as Hampshire and Isle of Wight Healthcare NHS Foundation Trust, will be formed.

Our new Trust will continue to cater to the unique needs of different communities, making healthcare across the county more accessible, as well as offering staff more opportunities for career development, training and partnership working.
