Job summary

We are looking for someone who is creative, highly motivated, shows initiative and is passionate and committed to ensuring high quality services.

The applicant would be working alongside all members of the team but would need to be an experienced community mental health nurse. It would be beneficial if the post holder has experience working in a community mental health setting as they would be working alongside the RMN in the team to deliver advice and support to the care homes regarding:

You will support the Care Home Team in providing clinical advice and case management for residents living in Nursing and Residential homes in order to prevent inappropriate referrals to outside services and unnecessary hospital admissions. The Care Homes Team work within a rapidly changing environment and flexibility is essential for this role

The post holder will be required work with a high degree of autonomy and professional accountability as part of the Care Homes Team and will act as a resource to other community and primary care teams. This post requires you to effectively communicate with care providers and the wider MDT, in identifying those residents at greatest risk of hospital admission and homes that require the most support. You will need to have a good Knowledge base of Safeguarding and the role CQC plays in the care Home setting.

Main duties of the job

1.1The Senior Community Mental Health Practitioner will work as a core member of the Care Homes Team to support the provision of good evidence based practice within both Residential and Nursing homes in the south east locality.

1.2To play an important role within the care homes to proactively identify and work with people, including the services users, care home managers and care home staff, to provide coordination and navigation of care.

1.3To work collaboratively with wider services-acting as a central point of contact to ensure appropriate support is made available and that the delivery of care is safe and effective.

1.4To provide bespoke mental health education sessions to the homes. Demonstrate the ability to come up with new and innovative ways to improve how the team can best support the homes.
