Job summary

1-year Fix Term contract, 0.8 WTE (30 hours per week).

The Southeast Hants and Fareham and Gosport community Heart Failure Team are currently recruiting community Heart Failure Nurses to work alongside our award winning team, providing care and support for patient in either a clinic or home setting.

The applicant should ideally have a minimum of 2 years' experience of working in Cardiology and knowledge of the management of heart failure patients would be desirable but not essential as training will be given. You will need to have the ability to work independently and as part of a team. There may be scope for job share.

Main duties of the job

To deliver a specialist service providing specialist case management for patients with Heart Failure under the supervision of the Heart Failure Nurse Specialist (HFNS), coordinating care with partner agencies and key stakeholders with the aim of promoting well being and where appropriate preventing hospital admission by maintaining safe and effective care in the community setting.

*To be confident in discussing treatment options with HFNS, medical and other nursing specialists.

*To offer an individual service and psychological support to patients and carers, respecting their diverse cultural and ethnic backgrounds.

*To provide Heart Failure education and supervision for Patient Support and Primary Care Teams to enable practitioners to deliver evidence-based practice.

*To contribute to the ongoing promotion and marketing of the Heart Failure service to all key stakeholders including patients/carers.

*To assist in the monitoring, evaluation and audit the Heart Failure service quality and develop reporting mechanisms in order to meet nationally and locally set targets.

To assist in the maintenance and update of protocols according to national and local guidelines for the safe and effective provision of the Heart Failure service
