We have an opportunity for a qualified Senior Mental Health Practitioner to join our team in Southampton Central Community Mental Health Team, working with adults with severe or enduring mental health needs.

We welcome practitioners who are looking to develop in their career and have interests in developing their therapeutic skills. We would also encourage band 5 applicants who are looking to develop and progress to a band 6 through an individualised development programme.

All requests for flexible or part time working will be considered and we are happy to discuss a mix of remote and office-based working with the successful candidate.

This is a fantastic opportunity to work in central Southampton, a diverse, multi-cultural area which also has two large universities on its patch.

Main duties of the job

To provide a care coordination role as a member of a multi-disciplinary team, engaging, supporting and working with a caseload of service users with severe or enduring mental health problems.

To help service users manage their symptoms, deliver evidence-based interventions and give advice and support on issues such as housing, benefits, education, employment, medication and social activities in the local area.

To assess and monitor service users mental state, provide comprehensive risk assessment and implement a management plan, undertake crisis management and contingency planning.

To provide therapeutic interventions in line with the agreed care plan and risk assessment, utilising psycho-social interventions and work on improving physical health.

To work in collaboration with service users as experts in their own care, promoting the principles of recovery, choice and personalisation, which value and empower the service user.

The final combination of Southern Health and Solent Trusts is anticipated to occur in July 2024, when the new organisation will be formed.

Our new Trust will continue to cater to the unique needs of different communities, making healthcare across the county more accessible, as well as offering staff more opportunities for career development, training and partnership working.

Desirable

  • Additional qualifications in psychosocial, substance misuse or psychopharmacological interventions are desirable.

Experience

  • Previous experience of working with people who are homeless.
