Job summary
We are looking for experienced staff from a health and leisure background to support our new programmes of care. If you feel passionate about helping individuals regain their confidence following a change in their abilities then we would love to hear from you. You will be supported by experienced hospital and community therapists and will follow guidance and or complete assessments of individuals in their own or health care setting to maximise their return of function.
Full training and induction will be provided, you will receive regular 121s, be part of a new peer support programme and be changing the way we deliver NHS services.

Main duties of the job
Implement and design rehabilitation programmes for individual patients and groups as advocated by Physiotherapists, including home exercise programmes
Assist and conduct exercise/rehabilitation classes both onsite and off both independently and with assistance of Physiotherapists and Physiotherapy Assistant
Assist patients to complete exercise or rehabilitation programmes with physical assistance and or guidance as required. Such as practising standing from sitting or regaining confidence in walking.
To advice reablement carers of delivery of exercise programmes and appropriate progressions following reassessment.

The final combination of Southern Health and Solent Trusts is anticipated to occur in July 2024, when the new organisation will be formed.
Our new Trust will continue to cater to the unique needs of different communities, making healthcare across the county more accessible, as well as offering staff more opportunities for career development, training and partnership working.
