Job summary
We are looking for an enthusiastic Band 6 Discharge Liaison Practitioner to join our friendly multi-disciplinary team. You are keen to hear from you if you are a qualified nurse, social worker or occupational therapist.
We can offer an individualised program of support and professional development to work through with the encouragement from an existing motivated, positive, and experienced team. Informal visit to the team is welcomed.
Main duties of the job
To support the safe discharge of individuals who have been admitted to an acute mental health unit.
Providing a 3-month period of monitoring in the community with a clear care plan.
Reviewing mental health needs and providing focussed interventions to support that recovery.
Enabling a safe transfer of care to primary care as appropriate.
Referring for CMHT Care Coordinator allocation if longer-term mental health needs are identified.
Making onward referrals and/or signposting to appropriate agencies.
The post holder will be expected to work proactively in forging professional working relationships with multi-agencies.
Car driver is preferrable. You will be working 9am -5pm Monday to Friday and occasional weekend or bank holidays. We are flexible regarding working patterns and working hours.
