Our fully integrated community OneTeam in the beautiful Avon Valley locality of The New Forest (Fordingbridge and Ringwood) seeks experienced or newly qualified nurses to join our expanding team.

The teams work closely together, to support patients in our local community with routine and acute health care interventions that support our patients to live as independently as possible and manage their acute and longer term health needs.

As well as caring for people in their own homes, we also operate clinic environments for enhanced wound care and catheter care to enable ambulatory patients to have better flexibility in managing their needs. The team also supports community vaccination programmes, keeping our local communities healthy and safe.

No two days are the same in this large, friendly team. New team members are supported with a comprehensive induction programme to The Trust and to our locality, we have a great preceptorship programme for newly qualified nurses too!

Flexible working shift patterns seven days per week, between the hours of 8am and 8pm can be supported.

Main duties of the job

Working within Professional guidelines and accountability, the post-holder will have responsibility for the management of a defined community caseload (via Nursing, mental health, Physiotherapy or Occupational Therapy interventions) for a group of GP Practices as part of the Integrated Care Team.

The post holder will have responsibility for managing, co-ordinating and delegating work ensuring referrals are dealt with according to clinical need, holistic assessments and care plans are recorded in line with Trust and Professional record keeping standards and clinical care at all times meets acceptable standards of patient safety and quality outcomes.

The post holder will be required to work with a high degree of autonomy and professional accountability. A primary aim of the role is to prevent unnecessary admission of patients to an acute hospital and support timely discharge into the community. The post holder will also have a responsibility for re-enablement, rehabilitation, health protection and promotion programmes that improve and reduce inequalities.

The final combination of Southern Health and Solent Trusts is anticipated to occur in July 2024, when the new organisation will be formed.
