Job summary

Fareham ICT 1 are a busy happy team who welcome new staff members and encourage staff development and learning. We are seeking either an experienced nurse and/or newly qualified nurse to join us in providing care and treatment to adults living in the area who have long term or complex health needs.

The scope of your role will focus on supporting admission avoidance, early supported discharge, promotion of independence and assisting in the effective management of patients with long term or complex conditions. You will be working in a multi-disciplinary way alongside a range of primary care services including those from GP's, social and voluntary sector. You will be responsible for managing your allocated work on a day-to-day basis ensuring patients are prioritised according to clinical need and that assessments and care plans are recorded in line with the Trust's standards, this role will also help develop you to manage the co-ordination of a shift and supporting junior staff.

Our service runs from 0700-1700, 7 days a week and therefore this position requires flexible working and all requests will be considered.

Access to a vehicle and possession of a full UK driving license is essential.

Main duties of the job

Working within Professional guidelines and accountability, the post-holder will have responsibility for the management of a defined community caseload (via Nursing, Physiotherapy or Occupational Therapy interventions) for a group of GP Practices as part of the Integrated Care Team.

The post holder will have responsibility for managing co-ordinating and delegating work ensuring referrals are dealt with according to clinical need, Holistic assessments and care plans are recorded in line with Trust and Professional record keeping standards and clinical care at all times meets acceptable standards of patient safety and quality outcomes.

The post holder will be required to work with a high degree of autonomy and professional accountability. A primary aim of the role is to prevent unnecessary admission of patients to an acute hospital and support timely discharge into the community. The post holder will also have a responsibility for re-enablement, rehabilitation, health protection and promotion programmes that improve and reduce inequalities.
