Job summary

Seeking a motivated and inspirational Foundation Consultant Practitioner (fCP) to join our UCR, and virtual ward team delivering care to prevent avoidable hospital admissions. The fCP will deliver specialist practice working closely with the Consultant Practitioner (CP) and ACP's. Along with the CP the fCP will take a lead clinical responsibility for the patient caseload, and supporting trainee ACPs to develop the four pillars of practice. The fCP will take a lead role in clinical assessment of patients with highly complex acute and/or chronic disease presentations, making differential diagnosis and prescribing treatments as an independent prescriber if profession allows. We have 4 pathways

Rapid response within 2 hours focuses on preventing avoidable hospital admissions for patients with acute ill health conditions by providing advanced clinical assessments that place the person at the core, This includes implementation of treatment and appropriate monitoring whilst in crisis.

Falls Car & UCR van consists of a frailty specialist alongside a Paramedic or ACP and responds to referrals from 999 for patients at risk of hospital admission through loss of independence and/or falls.

Frailty Virtual Wards supports patients who would otherwise be in hospital to receive the acute care and treatment in their own home.

Working with the ED Frailty Team in, supporting same day turn around, UCR/VW continue the patient journey from ED to home to reduce re-admission

Main duties of the job

Along with the CP the fCP will take a lead clinical responsibility for the UCR and virtual ward patient caseload.

Leading the trainee ACPs to develop their four pillars of ACP expertise, providing mentorship, and clinical supervision.

Take a lead role in the advanced clinical assessment of patients who have highly complex acute and/or chronic disease presentations.

Including screening patients for disease risk factors and early signs of illness/deterioration, making a differential diagnosis and prescribing treatments as an independent non-medical prescriber or equivalent if profession does not yet enable independent prescribing.

This role requires being part of a divisional on-call rota supporting the wider teams.

Qualifications

Essential

  • Appropriate professional nursing or allied health professional qualification with post registration experience of at least 5 years.
  • Professional UK registration e.g. NMC / HCPC
  • An Advanced Clinical Practice MSc accredited by the Centre for Advancing Practice
  • OR
  • Successful recognition with the Centre of Advancing Practice via the ePortfolio (Supported) Route to demonstrate educational equivalence that all the capabilities of advanced level practice at master's level (level 7) have been met.
  • Maintained a portfolio of ongoing evidence or work based learning.
  • Independent non-medical prescribing or pharmacology level 7 profession relevant qualification
  • Teaching and Assessing qualification or equivalent/ mentorship qualification.
  • Able to demonstrate a good command of the English Language both written and verbal
  • Computer Literate in both data entry and communicating via IT - emails and report
  • Experience of leadership in a healthcare setting.
  • Evidence of significant professional and managerial development, including a comprehensive and relevant CPD portfolio.
  • Evidence of working towards academic level 8 and/or through competency-based assessment of application of level 7 working, with scope to develop towards level 8 competencies.
