Job summary

We are seeking a Consultant Practitioner experienced in Frailty & Rehabilitation to join our service, based in Petersfield Community Hospital. The service delivers a step up, and step-down rehabilitation facility for the SE Hampshire population. . Our aim is to provide individualized care tailored to the patients' needs, whilst promoting individuality and independence. We support patients and families with complex decisions around discharge plans and destinations. The service works in conjunction with the Virtual Ward and the Urgent Community Response service within Portsmouth & SE Division, as well as the Community Assessment Units whose focus is to support patients with frailty who are residing at home.

The Consultant Practitioner will, provide senior leadership to the team of Ward Specialty Doctors and be responsible for providing autonomous clinical expertise and leadership as part of a committed multidisciplinary team working around the needs of our patients.

The Consultant Practitioner will be required to provide clinical advice and guidance regarding the management of specific patient cases and will provide medical oversight for all patients within their care. The wards at Petersfield Community Hospital have on average 60 admissions per month, with an average length of stay of 21 days. We work closely with the acute sector in Portsmouth Hospitals where Southern Health host a Re-ablement & Flow team to support the realisation of "next patient, to the next available bed."

Main duties of the job

The post holder will be required to work automatously to:

Provide values-based leadership across the care pathway, services, and systems in complex and changing situations.

Develop staff potential, add to, and transform the workforce, and help people to learn, develop and improve (in and from practice) to promote excellence.

Develop a 'knowledge-rich and inquiry' culture across the service and system that contributes to research outputs and has a positive effect on development, quality, innovation, increasing capacity and capability, and making systems more effective.

Establishing expertise across the system and nationally by using consultancy approaches and opportunities that have maximum impact on practice, services, communities, and populations, and which add to and sustain workforce capacity and capability.

  • Professional UK registration e.g. NMC / HCPC experience of at least 5 years
  • An Advanced Clinical Practice MSc accredited by the Centre for Advancing Practice
  • OR
  • Successful recognition with the Centre of Advancing Practice via the ePortfolio (Supported) Route to demonstrate educational equivalence that all the capabilities of advanced level practice at master's level (level 7) have been met.
