Job summary
We are seeking two individuals to support the development of the new CAMHS transitions service. The pilot will run for 12 months initially, but the hope is that young people will be supported to transition from our inpatient settings to the next stage in their recovery and this will be an ongoing part of our services.
The successful applicants will have the opportunity to work within both inpatient and community services, supporting young people back into the community after having time within inpatient services. The staff will be based with the inpatient team and will work closely with the network including social care and educational services.
The team will be required to make clinical decisions relating to risk and the management of supporting a successful discharge back into the community.
The team will have the opportunity to support social and therapeutic interventions, providing support for the young person on their recovery journey.

Main duties of the job
The purpose of this role is to work as part of the multi-disciplinary team at Leigh House, and Core CAMHS Service supporting young people by providing therapeutic interventions and support as they are discharged and move between services.
The post holder will:
*Assist and support the service and Multi-Disciplinary Teams by participating and leading in the treatment and support of young people who have been inpatients in Leigh House or are at risk of requiring readmission to hospital.
*To lead in supporting young people who are being transitioned between inpatient CAMHS and adult mental health services.
*Working within the inpatient and community teams to develop programmes of support appropriate to individual young people.
*To lead the young person's integration from hospital to home, and back into outdoor activities/clubs, hobbies and educational provisions using the model of social prescribing. The post holder would be required to participate in the activities along with the young person.
*To support young people into the community from an inpatient setting, working with accommodation services and local authorities.
*To facilitate parental and professional highly specialist training on self-harm (FLASH) and suicide prevention as previously offered to schools, hospital and social care professionals.
