Job summary
We are looking for a versatile, enthusiastic individual to work with the existing Palliative Care Team at SHFT, helping to provide outstanding care to our service users and patients as they are at the centre of everything we do.
ABOUT THE TEAM
The Palliative Care Team has an established team of three Consultants and two Specialist Drs who provide medical input and leadership to the multi-professional, specialist palliative care services across inpatient hospice and two community settings. This post arises as a result of an impending consultant retirement and service reconfiguration. A consultant advert will follow shortly.
WHAT YOU'LL DO
As a Clinician your responsibilities will focus on the specialist medical support of the Community and Hospice teams. On-call responsibilities working on a 1 in 4 rota alongside the 2 consultants and Specialist Doctor, providing advisory telephone support to the Portsmouth & SE Hampshire ICP population both in Queen Alexandra Hospital and the community.
This post will primarily focus on clinical provision, which as in any Palliative Care team includes a significant amount of advice and education provision for community teams, general practice and other members of the Palliative Care services.
Main duties of the job
The post holder will:
*Provide robust evidence based care, advice and support across all three settings
*Provide specialist advice, support and information to patients and carers through direct consultations
*Liaise with GPs, community teams, in-patient unit, PHT consultants, CNSs, and managers of any service
When working on the Hospice ward
*Responsibility for leadership of the day to day medical care of inpatients in the Rowans Hospice.
*The role of ward admissions co-ordinator during the working day is managed by the Palliative Care Support Hub, which senior nursing and medical staff are rostered to support.
*Conduct at least one senior ward round each week and attend the Thursday Ward MDT meeting
*Advising Specialty Doctors (numbers vary) on medical management and supporting supervision of the trainees (currently 2xFY2 & 2xGPVTS posts) as appropriate to their experience.
When working in the community
*Attend twice weekly SPC Caseload Review Meetings. Undertake initial home visits and clinical re-views
*Support of Community Matrons and specialist nursing teams by means of advice, joint visits and education
*Outpatient assessments as appropriate
*Attend, where appropriate, GP GSF meetings
*Attend CCG TARGET (GP education) meetings to support EoL education.
Palliative Care Support Hub
*Each Senior Doctor and a number of the specialty Drs, Band 7 CNSs and above, support the hub for one or two sessions each per week. See indicative job plan.
