Job summary
We have an opportunity for a 6 month secondment with an option to join us permanently for the right person. We are looking for an Advanced Clinical Practitioner/Trainee ACP OT or Senior Occupational Therapist to cover during a period of planned sickness.
UCR is a rapid assessment, diagnostic and treatment service, which aims to reduce unscheduled admissions for people with complex health needs, exacerbation of long-term conditions and frailty.
We work closely with our System Partners to deliver excellence in care and there will be opportunities to develop your ACP skills, whilst also supporting our patients with your core underpinning OT knowledge to keep them safe and well at home. We are the proud owners of our award winning Urgent Community Response Van, and have several other exciting projects in the pipeline.
We are a very supportive mixed multidisciplinary team overseen by a Consultant Nurse Practitioner.
The service operates 8am to 8pm, 7 days a week, 365 days a year from a hub, located in-between Basingstoke and Alton. We operate long and short shifts and you will be expected to work 1 weekend in 4.

Main duties of the job
We are looking for an individual to work as an autonomous practitioner to provide a responsive, safe, effective and caring service to patients. You will complete initial history taking, clinical and functional assessment, diagnosis, treatment and evaluation of care, and demonstrate safe, clinic decision making and expert care. This will include assessment and diagnostic skills, for patients within their own homes with a variable caseload of complex patients within a multi-disciplinary team structure.
Along with using a medical model to assess, diagnose and prescribe for acute medical conditions, the clinician will use their core Occupational Therapy skills to adopt an holistic approach to consider methods and interventions to prevent recurrence of problems and find solutions for independent living. There is an expectation that they will work outside the boundaries of their professional qualification but within their specific competency.
The clinician will participate in, and influence efforts, across health and social care to shape multi-disciplinary pathways designed to support patient choice, improve quality of life, promote self-management and assure early intervention through proactive provision of care in or as close to the patients' own home as possible.
You will offer bespoke supervision to other therapy staff within the team and support the wider team to manage patients with complex therapy, manual handling and equipment needs.
