Job Title: Advanced Clinical Practitioner -- Registered Adult Nurse, AHP or ECP

Salary band: Band 8a

Hours: Full time 37.5 hours per week (part-time considered).

Location: One Team locality hub (Totton/Avon Valley) -with the ability to work flexibly across sites subject to service needs.

This is a new and exciting opportunity to work in the community-based Frailty Support Service and help to develop and deliver our urgent community response and virtual ward service. We are recruiting a Band 8a Advanced Frailty Practitioner, who will be a Registered Adult Nurse, AHP or ECP with an interest in frailty. You will provide clinical and team leadership and work across the 4 pillars of advanced clinical practice.

The Frailty Support Team service within Southern Health provides a unique model of care nationally. We support acutely unwell adults who are experiencing a sudden decompensation in physical, psychological, or social well being in their own homes. Our primary aim is to provide first class health care in the home, giving our service users the choice to remain at home and recover more quickly.

We work as One Team with our community therapist, mental health, and nursing teams as well as GPs to offer tailored personalised care for our patients in each locality hub.

The Team is truly multi-professional, we currently have Consultant Practitioners, Physiotherapists, Occupational Therapists, Ambulance Technicians, Paramedics, Apprentice Nurses, GP's, Geriatricians and Administrators.

Main duties of the job

While we all work to our core strengths and abilities, we also help each other to develop new skills and acquire new knowledge that enables us to provide truly holistic patient centred care.

While the majority of working days will be spent in your usual base, there is some requirement to work flexibly from time to time from our other bases. There are five in total, located in Eastleigh, Totton (Southampton), Fordingbridge, Romsey and Lymington. We provide a service to our patients 7 days a week between the hours of 08:00 and 20:00 and weekends are worked in rotation.

Successful applicants will receive a comprehensive induction into the team and the trust. We have regular inhouse training sessions and access to a wealth of internal training and external courses and conferences to maintain your clinical skills and develop you academically. There is the opportunity to undertake MSc modules leading to becoming an Advanced Clinical Practitioner.

  • Appropriate professional qualification e.g. Registered Nurse, Occupational Therapist, Physiotherapist
  • Professional UK registration e.g. NMC / HCPC
  • An Advanced Clinical Practice MSc accredited by the Centre for Advancing Practice or Successful recognition with the Centre of Advancing Practice via the ePortfolio (Supported) Route to demonstrate educational equivalence that all the capabilities of advanced level practice at master's level (level 7) have been met.
  • Maintained a portfolio of ongoing evidence or work based learning.
  • Independent non-medical prescribing or pharmacology level 7 profession relevant qualification
  • Teaching and Assessing qualification or equivalent/ mentorship qualification.
  • Able to demonstrate a good command of the English Language both written and verbal
  • Computer Literate in both data entry and communicating via IT - emails and report
