Job summary
An exciting opportunity has arisen within Southern Health Foundation Trust to appoint an enthusiastic Therapy Assistant (Band 3) to work within the multi-disciplinary Integrated Care Team in Romsey. The job will include home visiting: providing interventions to patients within their own home and also in a clinic environment, as well as lone working without direct supervision. The scope of the role will focus on supporting admission avoidance, early supported discharge and promotion of independence and assisting in the effective management of patients with long term conditions and older people with mental health problems. The post would suit someone interested in integrated working and wanting to achieve the best for their patients. They will work within a multi -professional team to provide individualised patient care maximising individual's health and social care. The caseload is very varied, and you will see patients with long-term conditions including neurological conditions, diabetes, palliative care, orthopaedic procedures, and patients following falls. An opportunity to upskill in Otago Falls prevention exercise programme and support the weekly Falls Prevention groups based in the local Leisure centre. Applicants must have access to a vehicle for work and in possession of a full driving licence is essential. Within the contracted hours to cover a Saturday 10- 6pm shift pattern supporting across the therapy teams with registered therapist on an average of one shift every 10-12 weeks.
Main duties of the job
1.ROLE PURPOSE
The Rehabilitation Assistant will work as an active member of a community based/inpatient team and will provide high quality, efficient and effective service to patients.
The post holder will assume responsibility for delegated aspects of healthcare delivery, working within a scope of practice under the direction/supervision of a registered practitioner. They will work within relevant legal and ethical frameworks and in accordance with organisational protocols and policies.
The Rehabilitation Assistant will be proficient, knowledgeable and competent. They will develop in their role through the acquisition of further skills, knowledge and competences, to be determined by the needs of patients, the service and the team.
