Job summary
This is a unique opportunity for a registered nurse to join a creative and dynamic multi-disciplinary health team who are passionate to work with people with learning disabilities. We strive to reduce the health inequalities faced they by supporting them to access mainstream health services wherever possible, usually with reasonable adjustments; and by delivering health interventions when their learning disability prevents them meeting their health needs independently or prevents them accessing other health services. This essential role helps to prevent diagnostic overshadowing and reduce the incidence of preventable deaths.
We are looking for a driven, caring and enthusiastic nurse who can manage their own caseload, working autonomously when required. Working with the multi-disciplinary team to deliver health interventions in a holistic and person-centred approach is central to this role. This post requires excellent clinical, inter-personal and communication skills to work with patients, their families/carers and other partner services. You will be required to complete assessments within core areas (complex health, mental health, behaviours that challenge, epilepsy, autism and forensic) of clinical practice and develop robust evidence-based care plans to inform your intervention(s) to meet the health goal of patient. You will need to demonstrate effective leadership skills, be organised, self motivated and have good computer/IT and presentation skills.
Main duties of the job
*To co-ordinate care for a specified group of patients.
*Be a member of an integrated inter-agency, multi-disciplinary learning disability team.
*Contribute to effective multidisciplinary working within the Learning Disability team and build effective liaison with community, primary, other secondary and tertiary services and agencies to meet individual need and service development.
*Manage a caseload of patients under indirect supervision and be accountable for assessing, interpreting, planning and implementing nursing treatment and care to patients within professional guidelines.
*Ensure that systems are used to incorporate any adopted models of quality assurance and governance into everyday practice.
*Act on behalf of, and report to the Charge Nurse as required.
