Pathway Co-Ordinator

Job summary

We are currently offering an exciting opportunity for a Pathway Co-Ordinator to join the Pathways Team at Antelope House in Southampton.

We are looking for 2 skilled and dedicated nurse's who have a passion for working with individuals who are experiencing mental health difficulties and their families to deliver high quality care and support.

As a Pathway Co-Ordinator, you will work with agencies involved in the care pathways of clients and liaise with care teams to coordinate access to relevant interventions, support and self-management for clients alongside the Crisis Team (CRHTT), Inpatient wards, Community Mental Health Teams (CMHT) as well as other health & social care providers

Main duties of the job

You will support the planning, coordination, and transfer of service users across care pathways, liaising with Southern Health Foundation Trust units and other NHS or private providers as required. It is important that you enjoy working in a busy environment, managing multiple priorities, planned & unexpected, being able to work to a level of flexibility, accountability, and autonomy under pressure as part of a team.

Working alongside the Pathway Team Leader you will also provide support and guidance to a small team of Band 4 Discharge Facilitators who undertake practical support to inpatient service users to overcome barriers to discharge such as accommodation and finances.

The role will entail working 9-5 Monday to Friday full time, Job share or Part time hours will be considered.

We have 1 full time position at 37.5 hours per week and 1 part time position at 22.5 hours per week
