Senior Mental Health Nurse Practitioner /AMP

At Fareham CMHT, we are a dedicated team of professionals located in the heart of the community of the population we care for.

Our CMHT is a lively and dynamic Multi-disciplinary team, consisting of Social Workers, Nurses, Psychological Therapists, Support Workers and Consultant Psychiatrists. Our work is recovery focused and partnership based. You will carry a caseload and coordinate their care and participate in the teams shared care function- supporting service users who need a higher level of clinical contact. As a senior clinician, you will work alongside other agencies to promote holistic recovery focused care as in addition to providing reports, quality improvement and supervising junior staff and running clinics where appropriate.

You will demonstrate that you are experienced in care coordination, assessments, risk formulation and management. You will provide evidence based interventions, medication management, provision of reports. You will have a sound knowledge of CPA processes and safeguarding.

You will need to be able to work autonomously in the community and use your own initiative.

Main duties of the job

To provide psychiatric interventions to a defined client group and their families within East Hants area, and specifically to help ensure the provision of a comprehensive service to those made most vulnerable by virtue of mental ill health
