Job summary

The Crisis Resolution & Home Treatment Team in Southampton supports individuals with acute mental health needs requiring intensive input to remain in the community. The team also works collaboratively with our colleagues on the wards at Antelope House to help reduce the amount of time patients remain in hospital by offering support via hospital at home.

Our service provides a variety or clinical interventions to collaboratively tailor our care plans to each individual service user. These include psychological interventions, medication support, prescribing, access to group work and cares support.

We are looking for an enthusiastic, grounded, yet innovative, Mental Health Support Worker who is committed to the principles of service user focussed care to become part of our team.

Main duties of the job

As a Support Worker in the team, you will be supported by the multi-disciplinary team to deliver intensive, recovery focussed intervention for individuals presenting with acute clinical needs as an alternative to hospital admission. You will hold positive communication skills, enabling you to be a role model for others, as well as positively representing Southern Health NHS Foundation Trust. You will also be able to liaise effectively with other services, both internal and external to Southern Health Foundation NHS Trust, retaining service user focus at the heart of any clinical decision making to ensure the best outcome.

We are a 24 hour service and the team work a range of shifts including weekends and nights.

Potential candidates should ideally have relevant mental health community experience. We are looking for a candidate who has excellent skills, communication and knowledge, and a positive attitude to the care of adults within the community setting.
