Job summary

We are recruiting for a Band 7 Extended Clinical Practitioner, who will be a Registered Adult Nurses/Allied Health Professional and have access to their own vehicle and be able to drive for work purposes.

The New Forest Frailty Support Team service within Southern Health run a unique model of care to provide Urgent Community Responses and Virtual Wards. We support acutely unwell adults at home, who are experiencing a sudden decompensation in physical, psychological or social well being and are at risk of hospital admission. Our primary aim is to provide first class health care, in the home, giving our service users the choice to remain at home and recover more quickly.

Main duties of the job

While the majority of working days will be based in your usual location, there is some requirement to work flexibly from time to time from our other four bases. These are located in Fordingbridge, Eastleigh, Totton and Romsey. We provide a service to the older adults we work with 7 days a week between the hours of 08:00 and 20:00. Weekends are worked in rotation, approximately one in six, subject to the needs of the service.

Successful applicants will receive a comprehensive induction into the team and the trust. We have regular in-house training sessions and access to a wealth of internal training and external courses and conferences to maintain your clinical skills and develop you academically. Enhanced clinical practitioners will be emerging clinical leaders who are also involved in education, research and service development.

The team has service improvement and development embedded at its very core. As a team member you will be encouraged to help us shape and develop the care that we provide to people who access our service and their families.
