Job summary
An exciting opportunity has arisen for a Band 3 Support Worker working within the Early Intervention in Psychosis Team based at Cannon House. The role will involve working across the Southampton area. The work is varied and interesting and there is a strong team approach ensuring good support for the successful applicant. The role requires lone working, use of initiative and a commitment to team working and service user empowerment. There will be an opportunity to develop your skills working in a specialist team and to increase your knowledge of working with psychosis. Applicants will require experience of working in a mental health team. This post may involve weekend and bank holiday working on a rota basis as per service user need. Car drivers are required for this post due to covering a large geographical area.
Main duties of the job
To work with a caseload of Service Users on the Early Intervention in Psychosis Team caseload who require support with their mental and physical health using NICE recommended interventions for psychosis.
To support service users in the transition from Inpatient to Community setting.
To support service users who have experienced mental health problems who wish to return to work to find or retain employment or education by accessing benefits advice, liaising with work, education and training providers and by providing ongoing support to service users in employment and education.
To contribute to the opportunities for an individual's increased social inclusion by developing partnership working with voluntary sector agencies, and linking primary care initiatives.
To provide a physical and psychological care to service users in accordance with the specified plan of clinical care and in line with the recovery model.
To provide support to the person responsible for the clinical care of the service user.
To accurately record service user information.
