Job summary

An exciting opportunity has arisen for a Diabetes Specialist Nurse to join the North Hampshire Community Diabetes team within Southern Health NHS Foundation Trust. The Specialist team works to improve diabetes care in the North Hampshire community by working closely with primary care providers.

The successful candidate will work with the GP practices and the Integrated Care Teams within the North Hampshire system, providing leadership, a high level of clinical expertise in diabetes, innovation and implementation of support, education, and training of health care professionals. Alongside local DSN support, the post holder will be additionally supported by the consultant-led West Hampshire Community Diabetes team and work collaboratively with the ICS Diabetes and Medicines Management team in the continued development of the new Integrated Diabetes Service.

Main duties of the job

You will have a clear understanding of the challenges faced by Primary, Community, and Secondary care in diabetes. Alongside support of the local community care teams, you will work with the GPs and Practice Nurses to improve treatment to target rates, reduce admissions to secondary care, improve attendance at structured education, reduce variability of process and outcomes and drive innovation. The post is not case holding but will support primary care clinics and community care teams in a shares care approach which will include patient consultations. You will work with the diabetes leads within the ICS to develop and organise education events for people with diabetes.

There will be a requirement to be flexible, working in a range of settings and the ability to be forward thinking and cope with change. Ability to travel independently across the geography to fulfil the requirements of the post is essential.

The successful candidate will have excellent interpersonal, communication and presentation skills. Experience with online diabetes and/or primary care notes systems (EMIS, RIO) and other clinical packages e.g. ICE, would be an advantage but not essential as all training can be provided.
