We are seeking registered nurses to join our East Hants school nursing team in Alton.

Some of our great innovations include the introduction of ChatHealth, our 5-19 Text Message Advice Service for parents and ChatHealth for young people aged 11-19, PIP, our parent portal and the emotional health and wellbeing pathway for school aged children.

We are looking for someone with an inspirational and motivational personality and someone who is highly skilled in communication, with a strong approach to teamwork. The individual will be someone who is able to deliver appropriately delegated commissioned elements of the Healthy Child Programme (HCP) (DH 2009) as a member of the 0-19 public health nursing service. Southern Health NHS Foundation Trust is also an accredited Baby Friendly Initiative organisation.

Working as a team player, your role will include identifying health needs with children and families and in agreement with the Specialist Community Public Health Nurse (SCPHN), plan, deliver and review appropriate and evidence-based interventions to improve outcomes for children and families.

Main duties of the job

The primary role of the Community Staff Nurse (CSN) is to support in delivering the commissioned elements of the Healthy Child Programme (HCP) (DH 2009) as a member of the 0-19 public health nursing service (Health Visiting / School Nursing).

They will provide responsive, holistic, quality, family centred public health outcomes in line with the 4-5-6 model of the Healthy Child Programme including the high impact areas.
