Southern Health NHS Foundation Trust has an exciting opportunity for a Band 5 Nurse or Allied Health Care Professional to join the Learning Disability Health Facilitation Team. The post is a full-time substantive post covering the areas of Alton, Basingstoke, Winchester, Andover, Eastleigh Totton, the New Forest and Lymington.
This is an excellent opportunity if you are passionate about working with people with a learning disability, their families and carers, health services and wider organisations on a shared goal of ensuring that people with a learning disability have equal access to healthcare.
This role is critical in supporting healthcare services in delivery of best practice but also ensure that people with a learning disability, their families and carers are heard and supported appropriately. The overall aim of the team is to reduce health inequalities people with a learning disability experience and the incidence of preventable deaths.
The role will be delivered in the community, in various settings, for example, day services, GP practices or a team base. Working from home can also be agreed when appropriate.
The successful applicant will be proficient, knowledgeable and competent. Educated to Degree or Diploma level in nursing or allied health professional registration, or equivalent, they will continue to develop in their role through the acquisition of further skills, knowledge, and competencies and to be determined by the needs of patients, the service and team.
Main duties of the job
The Learning Disability Health Facilitator will work as an active member of the Learning Disability Health Facilitation Team to improve access to mainstream health services for people with a learning disability living within Hampshire
The post holder will assume responsibility for delegated aspects of healthcare delivery, working within a scope of practice under the direction/supervision of the Team Lead, Strategic Health Facilitator. They will work within relevant legal, ethical, competency and clinical frameworks in accordance with Southern Health NHS Foundation policies and procedures.
The Health Facilitator must hold a professional registration, for example, be a Registered Nurse or Allied Health Care Professional.
If undertaking a period of preceptorship, they will work under the direction and clinical supervision of a nominated preceptor. Once complete, the post holder will supervise, assess and teach qualified and unqualified staff including students.
The Health Facilitator will continue to develop in this role through the acquisition of further skills, knowledge, competencies and training, which will be determined by the needs of the service users, the team, and the wider service.
