Job summary
We have an exciting opportunity in our West CMHT team in Southampton, based in Shirley
In the CMHT, we are passionate about providing a service of the highest quality that is safe, effective, compassionate and responsive. You will be joining one of the 3 experienced and supportive teams, working with adults with severe or enduring mental health needs.
The successful candidate will be enthusiastic, with a recovery focussed approach. You will be supporting service users in designing support plans, identifying their needs and strengths, sourcing support and accessing the funding to which they are entitled. You will be monitoring and reviewing their social care needs and closely liaising with a variety of services, including Southampton City Council and Southern Health inpatient units, as well as outside agencies.
Main duties of the job
We welcome practitioners who are looking to develop in their career and have interests in developing their skills.
This is a fantastic opportunity to work in Southampton, a diverse city with some social deprivation. Knowledge of the Care Act and an interest in these issues would be advantageous.
