Job summary

An exciting opportunity has arisen in the Winchester District Community Teams.

We are seeking to recruit an experienced full time Band 6 enhanced wound care nurse to work in our community teams. The successful candidate will be able to deliver evidence- based nursing care in a supportive team environment.

You will work alongside Clinical Nurse Specialists in Tissue Viability and other enhanced wound care nurses. The successful candidate will act as an expert resource to community and care home staff. The applicant will have a willingness to participate in some TV education programmes with other team members. The role also provides support to an established TV Link Nurse cohort in their geographical area.

Your main role will be working within the community nursing teams in the locality to support registered and unregistered staff to achieve competence, skill and experience with Tissue Viability.

You will be expected to maintain clinical skills and competence, you will have the ability to work unsupervised, be able to prioritise your workload, have excellent communication skills and the interest and enthusiasm to promote best practice in tissue viability. As a team player, you'll be able to prioritise your work in order to meet deadlines and have a customer focused approach.

The successful candidate will need to be flexible in this role to include supporting the community team with weekend working.

Applicants must have a full driving license and full time access to a car.

Main duties of the job

The primary role of the Enhanced Wound Care Nurse is to work across a designated geographical area, providing support in the delivery of evidence based high quality specialist wound / skin care, advice and education to a range of staff within the Integrated Community Winchester District.

The Enhanced Wound Care Nurse in conjunction with a wide range of clinical colleagues and specifically, GP's, Practice Nurses and Social Care professionals, will lead and facilitate a patient or client focused, co-ordinated case management approach across primary and secondary care for people With complex tissue viability issues.

To undertake a comprehensive competence programme that will ensure high levels of knowledge and skill in the area of tissue viability care across the Community team.

To work in clinic settings, patients' homes and in residential care settings to support general clinical staff in providing tissue viability care

To act as a role model and expert in the prevention, treatment and care of tissue viability conditions

The will participate in and influence efforts across health and social services to shape multi-disciplinary pathways designed to support patient choice, improve quality of life, promote self-management and assure early intervention through the proactive provision of care in or as close to the patient's own home as possible.
