Job summary
The 2023 Skills for Health, Health Heroes winners, Urgent Community Response / Virtual Ward team are seeking an enthusiastic and forward thinking Extended Clinical Practitioner to join our team. This is an exciting new role for nurse or AHP who excels in clinical practice.
The primary focus of the role is to work as an extended clinical practitioner focusing 100% clinically in the assessment and review of acutely unwell adults in their own homes advanced, autonomous practitioner providing patient-centred clinical care, encompassing the skills of assessment, examination, diagnosis and treatment of patients.
Main duties of the job
As an extended clinical practitioner you will assess and treat people with a range of acute conditions alongside long-term conditions that place them at high risk of hospital admissions. You will implement a person-centered plan (including rehabilitation) to enable that person to stay at home, recover, and re-engage with the important aspects of their life.
The role allows you to implement a breadth of interventions, while having the support of consultant & advanced clinical practitioners from and health care support workers. This rewarding role allows you the time and scope to explore what's important for each person and work with them to improve their health and well-being.
