This is a full-time Community Nursing position which is designed to contribute to avoiding hospital admission and maintaining patient's independence in their own homes throughout the Waterlooville locality. This is achieved by both routine visits but also responding to Urgent Crisis Response (UCR) calls which are a key aspect of the role going forward. The UCR process is a new initiative in which we work alongside South Coast Ambulance Service, offering acute visits with other members of the MDT, and using a community based skill set to try to reduce the need for an ambulance to attend and to prevent the patient being admitted.
Main duties of the job
The successful candidate would be required to actively participate in ICT/MDT meetings with other team members and the wider MDT disciplines, coordinating a plan of care and working autonomously within a scope of practice as a registered practitioner. They will be expected to be involved in care planning and identification of person-centred care delivered in the home setting. Responsibilities will also include line management, coordination of the shifts, allocating the caseload and managing the roster.
