Job summary
Petersfield & Bordon a diverse area with a vibrant community. We are looking for a motivated and enthusiastic Senior Community Mental Health Practitioner to join our local Older Peoples Community Mental Health Team. The post involves working in Petersfield & Bordon providing skilled assessment & treatment interventions to older people with mental health problems. The post is based across the area.
To be a successful applicant you will need to be a registered Mental Health Nurse or Social Worker with experience in the care of older people with mental health problems. You must have a genuine interest in working with older people and be able to demonstrate a real commitment to providing high quality and compassionate care to patients and their families. You will play an important role within our team as we aspire to provide excellent patient care and develop our services in line with national and local drivers.
There is a strong multidisciplinary focus within the teams and you will be integral to the implementation of programmes of care and evaluating and communicating effectively with other parts of the service. You will also be expected to provide supervision to junior members of the team and mentorship responsibility to support student nurse learning. A full driving license is required.

Main duties of the job
To act as care co-ordinator to an agreed caseload of service users with severe and enduring mental illness.
To undertake ongoing assessment, treatment and evaluation for service users known to the service.
